All residents requiring care must be assessed by the Aged Care Assessment Service (ACAS). This Service determines the level of care required and will issue a document stating this and the dates for which the approval is current. Referral to ACAS can be made by a Carer, General Practitioner, Community Service Group or Agency, Health Professional or ACAS themselves.
Berlasco Court also requires the completion of an Application Form, which is available from Reception. This application requires the provision of relevant information in respect of the applicant, including details of financial information. This information is treated in the strictest of confidence at all times. This form also includes basic information on the accommodation charges. This Application Form is now generic, so once the original is filled in, copies can be made to give to other facilities.
There is a waiting list of people wishing to access the services provided at Berlasco Court. A copy of the current ACAS Approval and Application Form is required before placement on the list. An ACAS assessment remains current for a twelve (12) month period from initial assessment. If you have not received access to a residential aged care placement prior to the completion of this period you will need to contact ACAS for a re-assessment.
